Please complete the Sponsorship Ad Form given to you (or print one from the link below) and mail to the address on the form. You may also scan the form and email (sponsorship@cdmbaseball.org) with your artwork (for the print ad) as well as your logo and web address that we can use for the sponsor page of our website.
